Making an Appointment

Begin your pre-trip planning for international travel with a visit to Beaumont's InterHealth® department. InterHealth will provide you with helpful information, required vaccinations and customized information to ensure your good health. We recommend you make an appointment at least 6 - 8 weeks prior to departure to your host country.

The link below will open a form that you can print, fill out and mail or fax to InterHealth. Mailing or faxing your forms allows the physician or nurse to give you the best and most customized information for you and your travels.
