Christopher J. Milback, M.D., MBA Chief, Department of Family MedicineDr. Milback received his Bachelor of Science degree in biology from Siena College in Loudonville, New York. He then earned his medical degree from Upstate Medical University in Syracuse, New York. He completed his residency in family medicine at Albany Medical Center in Albany, New York where he served as chief resident. His interests in teaching, resident education, pediatric and adolescent medicine as well as women's health led him to join the faculty at Beaumont Hospital, Troy in 2000. Dr. Milback completed the faculty development initiative at the University of Michigan in 2002 and served as assitant director in charge of obstetrics and women's health from 2001-2005. in 2005, he was appointed medical director for the Family Medicine Center with oversight of patient care and quality improvement. Dr. Milback was named chief of the department of Family Medicine at Beaumont Hosptial, Troy in 2009. He earned his masters in business degree in healthcare administration from Oakland University in 2010. Dr. Milback enjoys traveling with his family, reading historical literature, and participating in outdoor activities.
David D. Lick, M.D., FAAFP Program DirectorDr. Lick attended The Ohio State University where he received his Bachelor of Science degree in chemical engineering. He then went back to his hometown of Toledo, Ohio, and obtained his medical degree from the Medical College of Ohio. He completed his family medicine residency at Beaumont Hospital, Troy and also completed the Primary Care Faculty Development Fellowship at Michigan State University. After working in private practice for several years he rejoined the residency as full-time faculty in 2005 and became program director in 2009. He is currently pursuing his MBA through the Univeristy of Texas at Dallas. His professional interests include hospital medicine, sports medicine and procedures. Dr. Lick is also active as a team physician for the Troy Athens Redhawks. He enjoys playing golf and tennis, reading, traveling, spending time with his wife, daughters, family and friends, and of course, watching Ohio State football.
Srikala Yedavally-Yellayi, D.O., Osteopathic Director of Medical EducationDr. Yedavally attended Wayne State University and received her Bachelor of Science in Medical Technology. She graduated from Michigan State University College of Osteopathic Medicine. After completing an osteopathic internship at Detroit Osteopathic/Bi-County Community Hospital, she completed her family medicine residency at Oakwood Hospital, Dearborn, Michigan, where she also served as chief resident. She joined the Family Medicine faculty at Oakwood. During her nine years at Oakwood she had a special interest in domestic violence and was a physician volunteer at a shelter for women and children. Her other special interests are addiction medicine and osteopathic manual medicine. Dr. Yedavally is married and has three children.
Marie Baloga, MSN, MSA, FACHE Corporate Director Family Medicine ServicesAttended the University of Detroit Mercy where she received her Bachelor of Science in Nursing. She then obtained a dual Masters in Nursing Administration and Business Administration and is a Fellow of the American College of Health Executives. In her career she has served as a Director of Education and an Administrative Director with responsibilities in hospital operations, Aging Services, Community Relations, Marketing/Public Relations and Service Excellence. In January 2010 she assumed the role of Corporate Director of the Family Medicine Centers and Residency programs. Marie enjoys spending time with her family and friends. She loves to golf and enjoys biking, reading, boating, and traveling.
Jodie Eckleberry-Hunt, Ph.D., FacultyDr. Eckleberry-Hunt completed a Bachelor of Arts in psychology at Eastern Illinois University, a Master of Arts and Ph.D. in counseling psychology at Indiana State University. She completed a primary health care psychology post-doctoral fellowship with Michigan State University - McLaren Regional Medical Center in Flint in 2000. Her clinical specialties include geriatrics, end-of-life issues, adjustment to chronic disease and physician wellness. Dr. Eckleberry-Hunt enjoys health and fitness and traveling.
Cynthia L. Fisher, M.D., FacultyDr. Fisher attended the University of California, San Diego, where she received her Bachelor of Arts degree in biology. She then obtained her medical degree from the University of Michigan and completed her family medicine residency at Providence Hospital. After leaving private practice, Dr. Fisher served as assistant residency director at Wayne State University and completed the Primary Care Faculty Development Fellowship at Michigan State University. In 1991, she joined the faculty at Beaumont Hospital, Troy as associate director in charge of Residency Education. In 2005 she completed a fellowship for Program Director Development and served as Residency Director from 2005-2009. Dr, Fisher is active in her service organization (ZONTA International), plays the flute with a local flute choir, she enjoys music, travel, gardening and golf.
Diane Landosky, R.N., Nursing SupervisorDiane Landosy began her nursing career as a licensed practical nurse and returned to school where she obtained her nursing associate's degree at Delta College and then her Bachelor of Science degree in nursing from Saginaw Valley State University. She has worked in multiple departments of the hospital and in an outpatient setting to gain a well rounded knowledge of the nursing perspective. Diane has worked for Beaumont in Nursing Management since 1989 and joined the Family Medical Center as a nursing supervisor in 1999. She enjoys boating, fishing, reading and travel and spending time with her family.
Elie Mulhem, M.D., FacultyDr Mulhem attended Damascus University, Syria where he received his medical degree. He completed his family medicine residency at the University of Minnesota in Minneapolis, Minnesota. After practicing for three years in Houghton Lake, Michigan, he joined the staff at Beaumont Hospital, Troy in 1998, and is currently the director of women's health curriculum. He initiated and oversees a clinic for gynecology procedures at the Family Medicine Center. Dr. Mulhem and his wife enjoy hiking, traveling and spending time with their sons.

Oleszkowicz, M.D., FacultyDr. Oleszkowicz attended Alma College where he received a Bachelor of Science in Exercise and Health Science before going on to attend medical school at Wayne State University in Detroit. He then completed his residency in family medicine at the University of Wisconsin-Madison, where he served as chief resident. His professional interests include hospital medicine, obstetrics, preventive health and chronic disease management. This, combined with an interest in medical education led him to join the faculty at Beaumont Hospitals, Troy, in 2009. Dr. Oleszkowicz enjoys spending time with his wife, running, swimming, tennis and travel.
David J. Rodgers, M.D., FacultyDr. Rodgers attended Bob Jones University in Greenville, South Carolina, where he obtained his Bachelor of Science degree in pre-med. He obtained his medical degree from Wayne State University and completed his family medicine residency at Providence Hospital in Southfield, Michigan. He completed the Primary Care Faculty Development Fellowship at Michigan State University while teaching at North Oakland Medical Centers. In 1995, he joined the faculty at Beaumont Hospital, Troy as assistant director in charge of Medical Student Education. As Educational Eirector for the Family Medicine Center, he oversees the educational programs for the residency as well as working with medical students and Wayne State University School of Medicine. His interests include running, rollerblading, photography and music performance.
Jennifer L. Tucciarone, M.D. , Medical Director, Family Medicine CenterDr. Tucciarone attended the University of Nevada, Reno, where she recevied her Bachelor of Science degree in Health Science. She obtained her medical degree from Ross University prior to completing her family medicine residency training at Beaumont Hosptials, Troy, where she also served as chief resident. After graduating in 2007, she joined the faculty of Beaumont Hosptial, Troy Family Medicine Residency Program. Her professional interests include preventative medicine, hospital medicine and women's health. She enjoys hiking, golfing, watching movies, cooking and most of all, spending time with her husband and three sons.
Anne Van Dyke, Ph.D., Director, Behavioral MedicineDr. Van Dyke attended Western Michigan University for a Bachelor of Arts degree in education. Her doctoral degree in Educational and Clinical Psychology was earned through Wayne State University. Dr. Van Dyke served a fellowship in Primary Care Health Psychology at Michigan State University-Genesys, St Joseph Hospital in Flint. Dr. Van Dyke joined the faculty in 1994 as Director of Behavioral Medicine. Her health psychology interests include coping with medical illness, adjustment to chronic illness, and women's health. She enjoys traveling, reading, sports, and time spent with family and friends.
