Provide to Site Coordinator

• a letter from your Graduate Program Director/ Clinical Director stating that you are a student in good standing
• a copy of your certificate of liability insurance (either your own or from your school/program)
• a copy of your Michigan temporary limited license/ limited license (TLLP/LLP), if applicable
