And should the need arise, your family doctor can manage your care and treatment with Beaumont specialists and services.

At the Beaumont Family Medicine Center, St. Clair Shores, we're helping new doctors become exceptional. Though we operate like a private practice, we offer our patients a team approach to care. Our resident doctors work with seasoned faculty members and on-site specialists in obstetrics and gynecology, orthopedics and psychology. As proud partners with Beaumont Hospitals' family medicine residency program, our goal is to help residents gain additional expertise in the specialty of family medicine.

We are proud to announce that our office is a Patient Centered Medical Home (PCMH). The standards of being a PCMH are inclusive of Quality Measures, Patient Overall Experience, Health Information Technology and Practice Organization.

PCMH is an approach to providing comprehensive primary care for children, youth and adults. The PCMH is a health care setting that facilitates partnerships between individual patients, and their provider teams, and when appropriate, the patient's family.>

What This Means

The patient or family chooses a Primary Care Physician (PCP) with whom they feel comfortable to be their "Medical Home" for all aspects of their care. The patient or family will then use their chosen PCP for every aspect of their care including preventative and well-visits to visits for chronic conditions such as diabetes, asthma, congestive heart failure, as well as emergent, non-life threatening issues.

The PCMH model has shown that when a patient or family chooses a Primary Care Physician as their "Medical Home" for their entire healthcare needs, their individual health is better and healthcare costs are lower.

EMMI Program Available To Our Patients

What is EMMI and how does it work?

The Beaumont Family Medicine Center is excited to introduce our patients to the latest technology in personal health management. The interactive Emmi Program, uses audio, text and images to help patients make sense of their personal medical information.

How do I get started?

The patient is given a "prescription" for an Emmi profile specific to their medical needs. You are then able to view your secure, password-protected Emmi programs online, at home, at your own pace and as often as you choose. You can take notes and ask questions of your caregiver from within the program. You can even share your Emmi profile with family and friends, if you choose to do so.

EMMI empowers you, the patient, with the capability to be actively engaged with your health care provider by giving you the tools and education for support.
