How to Get Copies of Your Medical Records

You may request copies of your Royal Oak, Troy, or Grosse Pointe Hospital medical records by:

  • Completing the Beaumont Authorization for Disclosure of Patient Medical Information form and mail to the address on the form.
  • Royal Oak and Troy patients can send a signed and dated written request to Medical Information Services Department (MISD) Attn: Correspondence, 3601 W. Thirteen Mile Rd., Royal Oak, MI 48073.
  • Requests are handled on a first-come, first-served basis and normally completed within five days. There is no charge for records of a specific procedure/test if they are mailed by Beaumont to another health care facility, to a physician or the patient. However, MISD does charge a fee for copies of a patient's complete medical record.

Royal Oak patients can come to the MISD records pick-up window located at the South Tower Entrance to sign an authorization form or pick up copies of records. They should see the entrance attendant at the South Tower Entrance for parking directions.

Royal Oak Campus Map (PDF)

Troy patients can come to the MISD records pick-up window located at the Main Entrance to sign an authorization form or pick up copies of records. They should see the Valet attendant and will be directed to a short term parking area. If you are sending someone else to pick up your records, the person must have written permission from you to do this.

Troy Campus Map (PDF)

Grosse Pointe patients should come to the Medical Records Department to sign an authorization form or pick up copies of records. Patients who are picking up their medical records can park in the visitor parking deck just outside the main entrance. If you are sending someone else to pick up your records, the person must have written permission from you to do this.

For Royal Oak and Troy patients, questions can directed to the Correspondence section 248-597-2888. For Grosse Pointe patients, requests and questions can be directed to the Beaumont, Grosse Pointe Medical Records Department by calling 313-473-1625.

How to Request Copies of Your Films

Troy

  • You may request copies of your images (films or CD's) by completing the Beaumont Request/Authorization for Release of Images and calling 248-964-8603 or fax the release to 248-964-8640.
  • Please allow at least 24 hours to process the request. Be sure to specify the date and type of procedure performed.

When you come in person to pick up your images, we will request you to present identification (driver's license/picture ID).

Note: *If someone other than the patient is picking up the Images, that person needs to present ID and a note from the patient giving the representative authority to pick up the images.

The Troy Radiology Release Area is open Monday - Friday, 8 a.m. - 4:15 p.m.

Royal Oak

  • Complete the Beaumont Request/Authorization For Release of Images. Please allow at least 24 hours to process the request. Please specify the date and type of procedure performed.

Cardiology - Royal Oak

You may request copies of your Cardiology Images (CD) and/or report by completing the Beaumont Request/Authorization For Release of Images and calling 248-898-5191 or fax request with release to 248-898-6530. Please allow 24 hours to process the request.

If your physician is requesting the images, we will mail out requested information via DHL overnight to outside physicians and hospitals, unless they have a courier service that will come and pick it up.

If you are requesting your own report and/or CD by phone, we request that you come in person to the Heart Center on the first floor to pick up your cath report and/or CD and you can sign the release while you're here. If the CD is being picked up by someone other than the patient, that person needs to present ID and a note from the patient giving the representative authority to pick up the record.

If you are an inpatient calling from your hospital room prior to discharge, we will send the CD up in the tube system with the release form, and once signed, the floor will tube it back down to the Cath Lab.

Requests for Bone Marrow Report and Slides

Please allow 24 hours to have a pathologist properly identify the appropriate slides to be sent to the referring physician. Please call 248-551-0423 to notify us of your request. Please complete the Beaumont Authorization for Disclosure of Patient Medical Information. Please include date of bone marrow biopsy (or estimated date). Complete address and phone number of physician who is requesting report/slides.

If you would like, you may mail the form to us and your report and slides will be sent directly to the requesting physician by express mail.

If you prefer to pick up your report and slides, bring the form with you and give it to a staff member at the MISD Records Pick-Up Window located at the South Tower Entrance. Please see the Valet attendant for directions to short-term parking area.

Anatomic Pathology (reports, slides or blocks) - Royal Oak

Please allow 24 hours to process your request. Please complete the Beaumont Authorization for Disclosure of Patient Medical Information.

If you would like, you may mail the form to us and your report and slides will be sent directly to the requesting physician by express mail.

  • Type of Biopsy/ies including year
  • Institution material is being delivered to including address
  • Physician's Name

To pick up your report and slides, please bring the form with you and give it to a staff member at the MISD Records Pick-Up Window located at the South Tower Entrance. Please see the Valet attendant for directions to short-term parking area.

Anatomic Pathology - Troy

To request your records, please complete the Beaumont Authorization for Disclosure of Patient Medical Information. Please be sure to include the following information:

  • Type of biopsy, date and year
  • Where and to whom you the information is being sent (including address and physician name/phone number)
  • Date of appointment
  • Pick up date and time
  • Address where you want us to mail the materials
  • If someone else is picking up your records, the person must have written permission from you to do this.
